No sign-off with anything missing

Follow one safety inspection in a workshop bay: the tipper over the pit, the full item list, the brake test, a sign-off that waits for a missing rectification, the signed declaration and the record in each operator’s format.

Interactive walkthrough

How the walkthrough works

Six short chapters follow one safety inspection in a workshop bay. DVSA’s guide says each safety inspection record shows the operator, the date and ISO week, the vehicle, the odometer reading, the full item list and any defects, and a named, signed, dated roadworthiness declaration; records are kept for at least 15 months.

DVSA Earned Recognition asks for 100% complete and correct inspection records. In this illustration the sheet will not sign off while anything is missing.

Guided playback takes about a minute and a half. You can pause at any time, choose a chapter, or take each step yourself.

  1. Chapter 01 of 06

    Into the bay

Bay 1 side-on: an eight-wheel tipper lorry with a white cab and an ochre body stands over the inspection pit, with the technician holding a tablet by the cab and the brake tester beside the front wheels.
The tipper is over the pit, and the sheet opens with the operator, the date and ISO week, the workshop, the vehicle and the odometer.

Bay 1 at Workshop A, Branch 2. Operator B’s rigid tipper, fleet number T-27, is over the inspection pit. At 07:40 the workshop office sends safety inspection SI-2417 to the technician’s tablet.

The technician opens the inspection sheet. Its header fills in from the job: the operator, the date and ISO week (Tue 15 Sep 2026, week 38), the inspecting workshop, the vehicle and the odometer reading, 184,316 km.

Everything in this walkthrough is illustrative. The workshop, operators, vehicle, people, readings, times and dates are fictional.

  2. Chapter 02 of 06

    The full item list

Close to the lorry’s second axle: the offside mudguard hangs loose, with a copper defect tag on it, and the technician stands beside it with the tablet.
Every item on the list is checked, and the defect is photographed where it is found.

The technician works through the full item list: 14 items inside the cab, then 16 under the vehicle from the pit. Each one is marked on the tablet as it is checked.

At ground level the offside mudguard on axle 2 is loose: its bracket has cracked. The defect is recorded where it is found and a photo is attached. The list is finished at 09:15: 58 items checked, 1 defect open.

  3. Chapter 03 of 06

    The brake test

The brake tester beside the lorry’s front axle: the wheels on the rollers, the printout on the console and the technician with the tablet close by.
The brake test printout is photographed and attached, and the brake performance is recorded.

The front axle sits on the rollers of the brake tester. The laden roller brake test runs and the console prints the result.

The technician photographs the printout and attaches it to the sheet, and the brake performance assessment is recorded beside it: service brake 58%, secondary 31%, parking 22%.

  4. Chapter 04 of 06

    Blocked while anything is missing

The technician’s tablet with sign-off blocked: the header, item list and brake test are complete, and one defect shows no rectification recorded.
The sheet will not sign off while a defect has no rectification recorded.

The technician tries to sign off. The sheet refuses: the mudguard defect has no rectification recorded, so there is nothing to declare yet.

The defect goes to the workshop office with the part it needs. At 10:20 the office issues a new bracket from stores to Bay 1.

    Rectified and declared

The tablet showing the signed roadworthiness declaration, with the technician’s role and initials, the signature and the date, and the defect marked as rectified.
The rectification is recorded, then a named, signed and dated declaration.

The new bracket is fitted and the mudguard is secure. The rectification is recorded against the defect at 11:40, with a photo, by the technician who did the work.

Only then does the declaration open. The technician signs it on the tablet: named, signed and dated, Tue 15 Sep 2026 at 11:52.

The same record, produced in the format each operator asks for, and kept on record.

The signed sheet reaches the workshop office. The workshop controller produces the record in each operator’s own format: one PDF per inspection for Operator A, a weekly bundle for Operator B and Operator C’s own sheet layout.

Each pack holds the sheet, the brake test printout, the defect photos, the rectification and the declaration. The record is filed with T-27’s history and kept on record until December 2027.
